Documents required for the import of medical supplies

  • Application letter
  • Sales contract - copy
  • Invoice - copy
  • Certificate of Origin - copy
  • Quality Management System Certificates - copies (ISO 13485 or CE 93/42).
  • Electronic version of invoice (in MS Excel format) should be sent to  email address (Name of the both parties as well as invoice numbers should be mentioned in the topic)
  • ID – copy (for individuals)
  • VÖEN – copy (for individuals)

All copies presented to our center should be certified with stamp and signature of the applicant.
