Documents required for the import of medical supplies
- Application letter
- Sales contract - copy
- Invoice - copy
- Certificate of Origin - copy
- Quality Management System Certificates - copies (ISO 13485 or CE 93/42).
- Electronic version of invoice (in MS Excel format) should be sent to email@example.com email address (Name of the both parties as well as invoice numbers should be mentioned in the topic)
- ID – copy (for individuals)
- VÖEN – copy (for individuals)
All copies presented to our center should be certified with stamp and signature of the applicant.
